Duplicate accounts sometimes created

Sometimes when recomputing users, the following error message will appear:

When inspecting the user we can see two projections on the same resource (in reality, there is still only one account). Both can be opened and return the same data:

Can someone explain why this is happening and what I can do to clean up and prevent it from happening?
